CARRILLO STREET GYM USE AND EXPECTATIONS 1. Gym reservation fees shall be paid on the 1st of each month for the previous month's use. 2. Valid account credit card is required. 3. No refunds provided if gym user cancels reservation. 4. To avoid theft, do NOT leave valuable items unsupervised. 5. Participants, parents, family and friends are permitted in gym. Uninvited guests should be asked to vacate. 6. No items shall be stored in the gym or storage areas. Items left behind will be discarded. 7. Gym users are responsible for the following: o Replace equipment used o Replace chairs o Discard trash and recycling into containers o Close all doors o Turn off lights, heater, etc. before vacating o Report broken items to jcsmith@santabarbaraca.gov 7. Parking: o Parking lot is for City employees only o Free parking is available on local streets or 75 minutes in Anacapa City parking lot 1. Permittee agrees to defend, indemnify, and hold harmless the City, its oFcers, employees and agents (each an “indemniGed party”) from and against any and all loss, damage, liability, claim, expense (including defense costs and attorney’s fees) (collectively “liabilities”) arising out of or in connection with the issuance or exercise of this Facility Rental/Use Permit Agreement, to the extent permitted by law, except liabilities arising from the sole, active negligence or willful misconduct of an indemnified party. 2. Permittee agrees to comply with all Conditions of Use stated above. Violation of Condition of Use may result in immediate termination of the activity authorized by this Facility Rental/Use Permit Agreement. This Facility Rental/Use Agreement does not authorize the sale of alcoholic beverages. A separate permit, including a permit issued by the California Alcoholic Beverage Control Board is required for sale of alcoholic beverages. 3. Permittee agrees to comply with all orders of the monitor or park ranger. 4. Permittee agrees to leave the facility in a good, clean condition after the event. Permittee agrees to liable to and pay the City for any damage to the facility or the site arising from or caused by the activities authorized by this Facility Rental/Use Agreement. 5. Permittee agrees that that Permittee has read, understands and agrees to comply with the terms and conditions of this Permit and the City’s rules, regulations and policies concerning the use of City park facilities, and understands and accepts the City’s refund policy.
